MAIB reports of two fatalities overboard...
The MAIB has published two fatality reports today, both as a result of falling overboard.
In the first case, the loss of the skipper of Reflex 38 yacht LION, 14.5 miles south of Selsey Bill on 18 June 2011 may not initially appear to have relevance to the motor yacht crew, there are as ever lessons to be learned particularly with respect to safety briefings, man overboard training, incident management and fatigue.
In the second case, a young fisherman was lost from fishing vessel ABOUT TIME off Pembrokeshire on 14 June 2011. I am sometimes slightly critical of conclusions that talk about formal risk assessments for fishermen ('the real world' etc), but this was avoidable and again, there are lessons for us all. http://www.maib.gov.uk/publications/investigation_reports/2012/about_time.cfm Please be guided accordingly.Back
