MAIB report on two fatalties overboard mv TIMBERLAND Nov 2012
Please find attached MAIB report into two fatalities overboard cargo ship TIMBERLAND in the North Sea November 2012. Boatswain Mercado Agbing and AB Raymund Alcontin both volunteered to go on deck in heavy weather to secure mooring ropes. Although provisions had been made to provide a lifeline arrangement and lifejackets, these proved to be inadequate in a heavy sea.
Lessons to be learned which may impact on our trade are –
- Heavy weather checklists
- Master's misjudgement of wave conditions
- Over-reliance on inadequate safety gear.
Sadly, had the rope store been used, this accident would not have occurred.
