Incident sharing - explosion on chemical tanker
A number of addressees have expressed interest in these reports, so my apologies to those who are looking for direct relevance to their work. Just for once, an accident that has almost no apparent synergies with the large yacht industry, being almost entirely tanker specific. Nonetheless, this was a terrible loss of a young man's life and there are always lessons to be learned. In this case, there is some thought to be given to safety awareness, acceptance of unusual conditions without question and - although not mentioned in the report, some complacency.
I would usually ask that this is raised at routine HESS meetings for general discussion, but the lack of immediate relevance to yachts may lose the interest of younger or less experienced crew members.
